Periodontitis and Chronic Kidney Disease Associations: Results From Systematic Review

Objectives: Chronic kidney disease (CKD) inclusive of renal insufficiency and failure, and periodontal disease (PD) each pose large health burdens globally. The purpose of this review was to quantify the strength of association supporting a bidirectional association between PD and CKD.
Methods: A systematic review of literature was performed for articles involving adults aged 18 years or more with clinically-defined PD or adults with CKD defined as glomerular filtration rate ≤59ml/min/1.73m2. Electronic searches of the PubMed, EMBASE, Web of Science, Scopus and Cochrane Library databases were conducted. Hand searching of reference lists was also performed. Effect estimates in each study were pooled and/or converted to odds ratios (ORs) for meta-analysis. Meta-regression and subgroup analyses investigated study heterogeneity. All analyses were performed using the software Stata 13.1.
Results: Twenty-seven studies were selected from an initial 4,055 abstracts sourced. Of these, 15 studies comprising 17,580 people considered CKD as an exposure for PD. Pooled estimates showed that individuals with CKD had OR 2.15 (95% CI 1.22—3.79) of having PD. Crude estimates which contributed 81% of the pooled weighted estimate were not significant: OR 1.61 (95%CI 0.89—2.91 ). Twleve studies comprising 146,789 people investigated PD as the exposure to CKD. The likelihood of having CKD was almost 60% higher: pooled OR 1.57 (95% CI 1.20—2.06). Adjusted weighted estimates in this instance contributed 92% of the pooled weighted estimate: OR 1.65 (95% CI 1.41—1.92).
Conclusions: This systematic review with meta-analysis confirms a bi-directional association between PD and CKD. Interventional studies examining the effect of periodontal treatment on renal function in patients with CKD is warranted.
