Hyperglycemia and Inflammatory Markers: Saliva, GCF, and Serum levels

Objectives: The objective of this study was to investigate the correlation in cytokine/chemokine levels between saliva and gingival crevicular fluid (GCF), and saliva and serum samples with regards to glycemic level.
Methods: The sample was derived from a clinical study, and consisted of 60 subjects diagnosed with (n=45) and without (n=15) chronic periodontitis, and HbA1C levels within (n=30) and above the normal range (n=30). Chronic periodontitis was defined as PPD 5 mm in 10 teeth, and BOP in 30% of sites. Biological samples were collected before and after nonsurgical periodontal treatment. GCF from the 4 deepest sites per quadrant was collected using paper strips and pooled; serum and unstimulated whole saliva samples were also obtained. Cytokine/chemokine levels were determined using the Ultrasensitive 10-Plex including IL-1β, IL-2, IL-6, IFN-γ, TNF-α, GM-CSF, IL-4, IL-5, IL-8, and IL-10.
Results: Statistical interactions for glycemic status were observed in the association between saliva and serum analytes levels; correlations being of different directions for normo and hyperglycemic subejcts. Whereas in normoglycemic subjects a positive correlation between saliva and serum levels for IL-10, GM-CSF, IL-5, INF-γ, TNF-α and IL-2 was observed (p<0.05), a negative correlation was found for IL-1β and IL-2 in hyperglycemic subjects (p<0.05). No statistical interactions for glycemic status were observed in the association between GCF and saliva samples; significant correlations between saliva and GCF levels for IL-1β and IL-6 were observed irrespective of the glycemic status (p<0.05).
Conclusions: Saliva predicted serum levels of cytokines/chemokines in normoglycemic, but not in hyperglycemic subjects; saliva predicted GCF levels of IL-1β and IL-6 irrespective of hyperglycemic status.
