At-home vs. in-office bleaching: a systematic review and meta-analysis

Objectives: A systematic review and meta-analysis were performed to compare the risk and intensity of tooth sensitivity (TS) as well as the bleaching efficacy during in-office and at-home bleaching.
Methods: A comprehensive search was performed in the MEDLINE via PubMed, Scopus, Web of Science, LILACS, BBO and Cochrane Library and SIGLE without restrictions. The IADR abstracts (1990–2014), unpublished and ongoing trials registry, dissertations and theses using the ProQuest Dissertations and Periódicos Capes Theses Databases were also searched. We included only randomized clinical trials. The risk of bias tool of the Cochrane Collaboration was used. Meta-analyses were conducted with the Mantel Haenszel method for random effects.
Results: After duplicates removal, 1139 articles were identified. After title and abstract screening, 29 studies remained. Fifteen studies were further excluded while 12 studies remained for qualitative analyses and eight for the meta-analysis. Nine studies used 10% carbamide peroxide with different concentrations (15% to 32%) for at-home bleaching. In-office bleaching was generally performed with 35% hydrogen peroxide (HP); three studies employed 38% HP and two studies employed 25% HP. The daily time use of the at-home bleaching varied from 3 min to 10 h; the number of days from six to 28 days. The protocol of the in-office bleaching was quite variable. No significant difference in the risk (odds ratio [95% CI] 2.186 [0.63-7.53]) and intensity of TS (standardized mean difference 0.823 [0.42-2.09]) as well as color change in ΔE* (standardized mean difference [95% CI] – 0.260 [– 0.77-0.22]) was observed.
Conclusions: In an overall comparison no significant differences in the risk/intensity of TS and color change were observed between bleaching modalities. This comparison, however, does not take into consideration variations in the protocols (daily usage time, number of bleaching sessions, and product concentration) of the bleaching techniques in the studies included.
