Clinical Performance of Posterior Bulk Fill Composite Restorations over 3 Years
Objectives: The aim of this prospective clinical trial was to evaluate the clinical performance of the bulk fill composite Tetric EvoCeram Bulk Fill (Ivoclar Vivadent) in posterior cavities in combination with an experimental adhesive using selected FDI criteria and a semi-quantitative clinical evaluation method (SQUACE). Methods: A total of 35 cavities (14 Class I and 21 Class II; 25 in molars, 10 in premolars) were restored with Tetric EvoCeram Bulk Fill using an experimental self-etch adhesive after conditioning with phosphoric acid. Restorations were placed in 28 subjects by three experienced clinicians. The follow-up examinations took place after 1 week (baseline), 1, 2 and 3 years. Two drop-outs were recorded after one year due to a change of residence. After 3 years, 33 (94%) restorations were assessed by means of selected FDI-criteria for the evaluation of direct restorations (Hickel et al. 2007). The FDI-criteria included the evaluation of aesthetic, functional and biological properties. Criteria concerning the restorations’ marginal quality were semi-quantitatively evaluated as % of total margin length (SQUACE method). Results: After 3 years, 100% of the restorations that were available for evaluation were still in place and the majority of them were graded either “excellent” or “good”. Documented marginal flaws affected only small portions of the total margin length (96.1% "perfect", 3.9% "good") and none of the cases examined showed marginal discolorations. Although the system comes in only three universal shades (IVA, IVB, IVW) the restorations exhibited a perfect color match in 69.7% of the cases and a "good" aesthetic integration in 30.3% of the restorations. Surface luster was still "perfect" in 63.3% of the cases. Minor loss of gloss or surface porosities were predominantly limited to occlusal contact areas and the surface was still rated "good" in 18.2% of the cases and "sufficient" in 18.2% of the restorations. More details are shown in Table 1. Conclusions: Tetric EvoCeram Bulk Fill shows a reliable clinical performance over 3 years without any failures, excellent margin quality and favorable aesthetic integration.
