Clinical and Biochemical Evaluation of Full-Mouth Disinfection in Generalized Aggressive Periodontitis Patients
Objectives: To compare the clinical and biochemical efficacy of quadrant scaling and root planing (Q-SRP), full-mouth disinfection (FMD) and full-mouth scaling and root planing (FMS) in patients with generalized aggressive periodontitis (GAgP) over 6-months period. Methods: Forty-two GAgP patients were randomly assigned into 3 groups. Q-SRP was performed in a quadrant-wise manner at 1-week intervals, FMS in 2 sessions within 24 hours, and FMD in 2 sessions with application of chlorhexidine to the intra-oral niches within 24 hours. FMD group also used daily 0,2% chlorhexidine for 2 weeks. At baseline, 3 and 6 months clinical parameters consisting of plaque index (PI), gingival index (GI), probing depth (PD), bleeding on probing (BOP) and clinical attachment level (CAL) were recorded and gingival crevicular fluid samples (GCF) were collected. GCF levels of IL-1β and IL-17 were analyzed using ELISA. Results: PI, GI, PD, BOP were improved significantly in all groups at 3 and 6 months (p>0.05), but CAL only in FMD and FMS groups (p<0.05). Sites with deep PD had significantly better clinical outcomes at 3 and 6 months in FMD compared to Q-SRP (p<0.017). GCF volume decreased significantly compared to baseline in all groups at 3 and 6 months (p<0.017). GCF levels of IL-1β decreased at 3 and 6 months only in FMD (p<0.017) while there was no change in GCF levels of IL-17 at any time points (p>0.05). Among the groups there was no difference in GCF levels of IL-1β and IL-17 at any time points (p>0.05). Conclusions: FMD compared to Q-SRP showed better clinical outcomes in the treatment of GAgP during 6-months. Moreover, the effect of FMD on IL-1β levels was revealed.
