The influencing Factors in the Treatment of Mucosal Benign Pemphigoid
Objectives: The purpose of this study was to reveal the effects of different factors on the treatment of mucosal benign pemphigoid through a retrospective analysis. Methods: 48 patients from the Department of Oral Medicine of Peking University School of Stomatology were recruited in the study. The diagnosis of pemphigoid was made according to physical as well as pathological examination. The traciing time was more than 3 months. Results: 25% of patients were male (12/48) and 75% were female (36/48), with mean age of 58.9±12.5. The course of pemphigoid was 13.5±21.5 months. BP180 antibodies were positive in 30.6% (11/36). The initial decrement of glucocorticoid dose in 71.4% (25/35) of patients was observed within first 3 months. Pearson analysis showed that the oral lesion area of patients were positively correlated with age (p<0.05), suggesting that the older the patients, the more severe the disease. It was also found that the larger the oral lesion area, the more cumulative amount of steroid dose in the first three months, the more maintenance dose at sixth month and also at the 1st year. There was no correlation found between the initial dose of the hormone, the accumulated amount in first-3-month, the weight, the age and the level of BP180 antibody. Spearman analysis also showed that the point of dosage reduction was significantly positive correlated with the accumulation of hormone in first-3-month (P < 0.05). Conclusions: This study suggests that the size of the oral lesion in pemphigoid should be considered as the first influencing factor for treatment. The initial dose of glucocorticoid should be administrated enough so that cumulative amount required would be reached as soon as possible.
