Objectives: With the growing popularity of surgical implant guide systems, awareness about possible deviations in horizontal distance and angulation should be raised. This study discusses the accuracy of surgical guides used in oral implantology based on guided surgery software made from 360 imaging (Atlanta, USA) and Straumann CARES® Guided Surgery (Basel, Switzerland) of the same CBCT scan. Methods: CBCT scans of 6 mandibular study models with 4 guttapercha fiducial markers incorporated were taken using Planmeca ProMax 3D CBCT (Planmeca Oy, Helsinki, Finland). Six implants were planned in the Straumann and 360 Imaging software for each model. The implants had a planned distance of 7mm between each other. Based on the planning, 6 surgical guides were produced and used to drill 2mm diameter implant bed preparations into the models at 90° angles. After the drilling procedures, new CBCT scans were taken as well as new measurements at two distinct depths for holes drilled on the right and left sides of the mandibular arches and compared to the originally planned positions. The data was then compared to the virtually planned positions to determine the accuracy of the drill system. Results: 36 implants were planned with the Straumann system and 36 with the 360 Imaging system; in total 72 pilot drill preparations were executed. All of the drilling sites were found to have some deviation from the planned implant position. After averaging the error for each drilling site there was an average 2.81±1.32° angle error for drilling through the guided surgery system. There was also a 0.497± .257mm deviation from the ideal position of the implant. Conclusions: The clinician must consider position and angle deviation when planning and placing an implant with guided surgery in order to keep a safe distance from vital anatomic structures.
