Systematic review of longevity of posterior resin composite restorations in adult

Objectives: To conduct a systematic review of the literature on the longevity of posterior resin composite restorations in adults.
Methods: A systematic literature search was conducted with pre-determined inclusion and exclusion criteria. Prospective clinical studies with a minimum of four years of follow up, forty restorations per experiment group and a drop out lower than 5 percent per years were selected. Abstracts and full-articles were assessed first independently and then agreed on by five reviewers. The methodological quality of the studies was assessed according to the Swedish Council on Health Technology Assessment (SBU) standard checklist determining the extent to which the studies met the basic quality criteria
Results: In all 4275 abstracts were identified by the literature search, 93 articles were read in full text and eighteen studies met the criteria for inclusion. There were 80 failures of restorations with a total follow-up time at risk for failure of 62030 months. The overall incidence rate for all causes of failure were 1.54 lost fillings per 100 filling years. The most common biological reason for failure was due to secondary caries with or without fracture of the filling, in total 31 restorations were lost due to that cause. The quality of the evidence was limited (++00).
Conclusions: This systematic review confirms that the major reasons for failure are secondary caries and filling fracture. The overall survival proportion of posterior composite fillings was high
