Cone Beam Computerized Tomography Measurement of Alveolar Ridge Dimension at Anterior Maxilla for Implant Treatment Planning

Objectives: To use Cone Beam Computerized Tomography (CBCT) to measure the alveolar ridge dimension and degree of buccal undercut at the anterior maxilla to help develop implant treatment planning.
Methods: Sixty-two patients were included. Measurements were taken at the cross sectional views in the middle of the maxillary right canine, lateral incisor, and central incisor. Alveolar height was measured from alveolar crest to floor of nasal fossa. Alveolar widths were measured from buccal to palatal cortical plates at thirds from the alveolar crest to floor of the nasal fossa. Buccal undercut location was measured from where the buccal cortical plate starts dipping to a line extending at the alveolar crest that is perpendicular to the long axis of the alveolar ridge. Buccal undercut depth was measured from the deepest point of the undercut to a tangent line of buccal plate paralleling to the long axis of ridge.
Results: Mean alveolar width increased from coronal to apical for each tooth. Mean widths (mm) were: Central Incisor – 8.08, 8.69, 11.95; Lateral Incisor – 7.07, 7.34, 10.55; Canine – 8.92, 8.64, 11.22. Lateral incisors had significantly smaller alveolar width (<.05). No significant difference in ridge height was noted among the teeth. Undercut locations (mm) were: Central – 5.80; Lateral – 3.65; Canine – 5.29. Undercut Depths (mm) were – Central - 0.75; Lateral – 0.91; Canine – 0.73. Percentages of the presence of buccal undercut were: Central – 40%, Lateral – 73%, Canine – 37%. Ridge width and height did not correlate significantly with age or gender.
Conclusions: Lateral incisors most frequently had buccal undercuts, which were located most coronally. They also had the least alveolar volume for implant placement.
