Comparative Response of Peroxide Whitening: Strips or Brush+Pen

Objectives: This clinical study was conducted to compare the safety and whitening efficacy of two marketed, self-directed whitening products.
Methods: Institutional review and informed consent were obtained, and subjects with no previous history of teeth whitening were randomly assigned to high-adhesion 9.5% H2O2 tooth whitening strips (Crest® 3D White Whitestrips® Advanced Vivid) or Colgate® Optic White™ manual toothbrush with an integrated peroxide-based whitening pen. All products were dispensed in blinded test kits with anticavity toothpaste and marketed usage instructions. The Strip group was instructed to apply whitening strips for 30 minutes once daily over a 20 day period along with normal oral hygiene with a regular manual brush, while the Brush+Pen group was instructed to use each product twice daily for 28 days. Whitening efficacy was assessed instrumentally as L*a*b* color change from digital images of the maxillary anterior teeth at baseline, Day 15 and Day 29.
Results: Mean (SD) age of subjects was 39 (13.2) years. Relative to baseline, only the Strip group demonstrated significant (p < 0.0001) two-parameter (–∆b* & +∆L*) color improvement at Day 15. At the end of the study (Day 29), the Strip group exhibited significant (p<0.0001) color improvement. In contrast, the Brush+Pen group had significant (p<0.05) reductions in yellowness and lightness (more grey). Comparing treatments, the Day 29 adjusted mean (SE) for the Strip group was -1.85 for ∆b* and 1.09 for ∆L*, compared to -0.37 and -0.36 for Brush+Pen group, respectively. Treatments differed significantly (p < 0.0001) with the Strip group exhibiting greater two-parameter whitening at both time points. Treatments were well-tolerated, transient tooth sensitivity represented the most common adverse event, and there were no “for cause” dropouts.
Conclusions: This clinical research demonstrated superior whitening efficacy of a 9.5% H2O2 whitening strips compared to an integrated brush+pen peroxide whitening system.
