Impact of Chronic Periodontitis on Glucoregulatory Biomarkers in GCF of Adults with and without Type 2 Diabetes

Objectives: To compare the levels of 10 diabetes-related biomarkers in the gingival crevicular fluid (GCF) of patients with type 2 diabetes presenting with and without chronic periodontitis. The concentrations of the biomarkers were also investigated in systemically healthy subjects, with and without chronic periodontitis.

Methods: The material comprised a total of 152 subjects, stratified according to type 2 diabetes and periodontal disease as: 58 type 2 diabetes subjects with chronic periodontitis (G1), 20 type 2 diabetes subjects without periodontitis (G2), 58 systemically healthy subjects with chronic periodontitis and (G3) and 16 systemically healthy subjects without periodontitis (G4). GCF samples were collected from each participant. The concentrations of the biomarkers were measured using multiplex biometric immunoassays. Periodontal pocket depths were recorded in mm. Subsets G1 and G2 and subsets G3 and G4 were compared independently.
Results: All the biomarkers except C-peptide were up-regulated in G1 compared to G2. C-peptide, GLP-1, insulin and leptin concentrations were significantly higher in G4 than in G3. The concentrations of ghrelin, GIP and glucagon were higher in G4 and PAI-1 and visfatin were higher in G3, but the differences were not statistically significant. The number of sites with pocket depth ≥ 4mm correlated negatively with C-peptide and positively with GIP and visfatin.

Conclusions: Among type 2 diabetes patients, biomarkers associated with the medical complications of the disease are up-regulated in subjects with chronic periodontitis. Comparison of systemically healthy subjects showed that chronic periodontitis adversely affects the GCF levels of biomarkers involved in the regulation of glucose metabolism.
