Method:
This retrospective, multicentre study was carried out by retrieving archival slides of cases diagnosed with OLP, OLL and Oral epithelial dysplasia (OED). A total of 165 cases, 70- OLP & OLL and 95-epithelial dysplasias were re-examined. The diagnosis of OLPs and OLLs were reconfirmed according to the modified WHO diagnostic criteria,2003. They were then verified for dysplastic changes. Among cases of leukoplakia diagnosed with epithelial dysplasia, we verified for the presence of lichenoid features.
Result:
A total of 70 cases of OLP and OLL were microscopically evaluated ( OLP-54; OLL-16). Dysplasia was noted in 11 cases ( OLP – 8/54; OLL- 3/16). 15 cases of OLP(15/54) were found to be OLLs as per WHO criteria. On screening the slides of OED it was found that 22 of the 95 cases showed lichenoid features.
Conclusion:
Krutchkoff and Eisenberg referred to OEDs showing dysplasia as LD. In reality it may not exist. Also, OLP is a potentially malignant disorder and there are possibilities for it to show dysplasia. Based on our findings that OED was found in OLP/OLL and OED had lichenoid features, we propose that these microscopic features may co-exist Presence of dysplasia in OLP/OLL may explain their malignant potential. Hence the present diagnostic criteria has to be modified in order to reduce confusion and controversies with regards to OLP.
