Osteorradionescrosis in Patients with Carcinomas: Recommendations for Clinical Practice

Objective:

To propose a recomendations guideline to prevent osterorradionecrosis (ORN) in patients undergoing radiotherapy for head and neck carcinomas in the National Cancer Institute.

Method:

Clinical Practice Guidelines that recovered information from relevant primary studies from MEDLINE and SciELO, LILACS quality assessed with an explicit methodology, Scottish Intercollegiate Guidelines Network (SIGN) and some items on which bases its recommendations for before, during and after radiation therapy to prevent the occurrence of ORN.

Result:

An selection of 42 studies published between 1998 and 2012: Cases and controls (2) Cohorts (1), prevalence (5), case series (17), case reports (4), narrative review (10), systematic review (1) clinical trial (1). which established the prevalence, associated factors and ways to prevent ORN. Thirteen were selected for their methodological quality to establish clear criteria for oral management recommendations for before, during and after radiotherapy. It took into account the preferences of patients and the considerations for implementing this guidance.

Conclusion:

Recommendations to prevent ORN in these patients provide clear benefits with some acceptable risks. The guidelines and settings proposed by this study are easy for patients to use.
